To Register an Adult

1. Download the registration form here
2. Complete and sign page 1, the liability release.
3. Chaperones and Volunteers: complete page 2. If not from Covington Diocese, attach proof of child protection compliance from home parish. We will verify that local adults are fully compliant with VIRTUS. VIRTUS
4. Calculate fee as shown below.
5. Mail form with fee as shown below.

To Register a Minor (age 13-17)

1. Download the registration form here
2. Parent/guardian must complete and sign page 1, the liability release. You MUST designate a chaperone.
3. Calculate fee as shown below.

Fees

1. $50 per Youth participant (ages 13 to 30); $40 per Adult chaperone or volunteer. EARLY BIRD DISCOUNT! If postmarked by Oct. 15, only $40 per youth, $30 per adult.
2. Make checks payable to Covington Youth Retreat Committee.

4. Fees are waived for some volunteers; ask your coordinator.

Mailing and Deadlines

1. Send all forms and fees to: Covington Youth Retreat Committee, PO Box 17955, Fort Mitchell, KY 41017-0955.
2. Youth groups can mail all forms together and pay with one check.
3. Oct 21: deadline for submitting forms by US Mail.

Walk-In Registration:

1. Doors open 5:30pm on Friday evening for walk-in registration — space permitting.
2. Registrations accepted first come, first served — space permitting.
3. No admission without all forms fully completed and signed.
4. Minors not admitted without qualified chaperones.
5. Chaperones not admitted without proof of child protection compliance.
6. Make checks payable to Covington Youth Retreat Committee.
